We’re currently in the phase where HWC received our provisional license, & we are currently admitting 10 patients for pro bono home health services.
PERSONAL CARE SERVICES
Adult Family Care
AFC is provided in a home to up to 4 individuals at any given time. Individuals live in the home with an unrelated caregiver and are provided with socialization, meals, supervision and hands on assistance with ADLs/IADLs, and medication oversight, while still maintaining appropriate independence. Care is provided 24/7; individuals are not to be left alone for any period of time.
Home Delivered Meals
HDM is a nutritionally balanced meal delivered to an individual’s home. HDM may include:
- Diet and nutrition counseling provided by a registered dietician
- Nutritional education based on the needs of individuals
- Diet modification according to a physician’s order to meet an individual’s medical and nutritional needs such as diabetic diet or low sodium diet.
Nutritional Supplements
NUTS include liquid supplements, such as Boost or Ensure, to support in maintaining an individual’s health, so they are able to remain in the community. Supplements must be ordered by a physician, physician assistant or nurse practitioner.
Light Housekeeping
Attendant Care
ATTC provides direct, hands-on care to individuals for the functional needs of activities of daily living (ADL). - Bathing and personal grooming such as oral hygiene, shaving, hand and foot care, intact skin care, application of cosmetics, dressing
- Transfers, ambulation and use of assistive devices
- Meal planning, meal preparation, and meal clean up
- Monitoring and assisting with safety such as waste disposal and household tasks - Provide medication reminders
- Transportation to nonmedical community activities
**ATTC cannot provide care for skilled needs such as specialized feedings, Hoyer lift, wound care, or ostomy care. Skilled need care requires an RN, LPN, or other health professional.
Home and Community Assistance
HCA provides assistance for instrumental activities of daily living (IADLs) to individuals in their home.
- Dusting and mopping vacuuming floors
- Straightening up furniture
- Cleaning kitchen, dishes, bathroom (including commode chair or urinal)
- Laundry - Changing linen and making beds
- Washing windows - Removing trash
- Meal planning and preparation
- Completing errands or unassisted transportation for nonmedical community activities
- Minor pet care
- Outdoor tasks including raking, snow removal, and lawn mowing.
Community Transitions Services
CT are funds available for setup expenses for individuals transitioning from an institution to their own home where the person is directly responsible for their own living expenses in the community. CT funds can be used for:
- Essential (non-luxury) furnishings such as bed, table and chairs, window coverings, one land-line telephone, eating utensils, housekeeping supplies, food preparation items, microwave, and bed or bath linens
- Safety assurances such as pest eradication, allergen control, one time cleaning
- Security deposits and application fees
- Setup fees or deposits for utilities or service access including telephone, electricity, heating, water and internet.
Non-Medical Transportation
Transportation service for individuals to be able to travel to nonmedical community services, activities, and resources.
There are 3 types of TRAN:
- Non-assisted: individual does not require a lift to transfer in and out of the vehicle
- Assisted: individual requires a lift to transfer in and out of the vehicle
- Adult Day Service Transportation: round trip transportation provided by ADS
**Transportation to medical appointments is not allowable under this service.
Grooming & Hygiene
Help clients maintain their appearance, which in turn promotes self-esteem. Includes washing and combing hair, shaving assistance, nail care (non-diabetic), and oral/denture care.
Dressing
For our elderly, as mobility decreases assistance with dressing may become an important need.
It's not uncommon for seniors to stop bathing regularly. Our Care Attendants encourage clients to maintain healthy and safe bathing habits, and assist them with the process.
